Consent and Authorization for Participation in Camp and for Medical Treatment // TEEN CAMP *

I, the parent or legal guardian, authorize BeMore Inc. (Ministries) Camp Staff to obtain necessary medical care or treatment for my child (named on this application) as recommended by a licensed physician. Every effort will be made to contact me prior to treatment, if possible. I release and hold harmless BeMore Inc. (Ministries), Silver Heights Camp, their staff, volunteers, and affiliates from all claims of injury, sickness, or damages that may occur during camp activities or travel. I grant permission for my child to participate in all camp activities, and authorize the camp to provide transportation, meals, and lodging as needed. I understand that all camp fees are non-refundable. If my child is dismissed, unable to attend, or if camp is canceled due to weather or other acts of God, all payments will be considered a donation to BeMore Inc. By signing below, I acknowledge this authorization and release, and understand that reasonable care will be taken to ensure my child’s safety during all camp activities.

I give permission for myself or my child to be transported to and from camp-sponsored activities in a church bus, rental, or private vehicle.

Discipline Release & Damage Liability Acceptance // TEEN CAMP *

In the event of misconduct, I authorize the staff to send myself or my child home at my expense. I acknowledge that all camp fees will be forfeited. Furthermore, any destruction of Be More properties, either real estate or equipment, or damage to District or Camp property, shall require reimbursement by those responsible for the act or by the parents or guardians of those responsible. In addition to reimbursement of repair/replacement costs, a fine of $200-$500 will be levied and must be paid within 30 days of the incident to BeMore Ministries.

I hereby give permission for any photos or videos to be taken during camp to be used for promotional purposes and any other use within reason by BeMore, Inc. aka BeMore Ministries. I understand that BeMore Ministries will not distribute or share photos or videos with any third-party affiliates or organizations beyond the purposes listed above.

Release of Liability // TEEN CAMP *

I voluntarily agree to allow myself or my child to participate in the activities at Silver Heights (Kids Camp location) along with BeMore Inc. (Ministries). I hereby waive, release, and hold harmless from any liability for damages or claims for personal property damage which may arise in connection with any and all hazards of these activities against BeMore Inc. (Ministries), Silver Heights and their staff, officers, agents, and employees. They will take every possible step to keep my child or me safe, and I accept the risk associated with camp activities, including serious injury or death.
